Helping you Live Well at Home
Our Eldercare services are a part of our Live at Home at Home Program. We worked with well over 700 older adults in central Minnesota in 2011 through this program. The majority say their goal is to remain living at home. Through our Live Well at Home program and Eldercare services, we can help you do that.
We work with both individual adults and family members who are providing care for an aging parent or their spouse. (Click here for more information about our Family Caregiver Support services.)
Our Live Well at Home Case Workers:
- work with older adults like you, who usually live alone, and whose goal it is to remain at home
- identify any risks that could keep you from their goal
- design a plan to eliminate the identify risks so you can remain at home.
Our professional Case Workers are kind. They are also knowledgeable and resourceful, and can recommend services that can help you or your loved one to Live Well at Home.
As part of a pilot project originally developed in part with the MN Board on Aging, this model of care is highly effective at helping elders to Live Well at Home, and is now being replicated across the state.
Besides our professional services, we also coordinate volunteer services that can assist you with things such as getting to medical appointments or the grocery store; with light housekeeping or chore services.
